We provide:

• Individualized exercise program and risk factor assessment
• Group exercise classes
• Group and individualized education
• Communication with your doctor/nurse practitioner
• Mentorship for students and community partners
• Keynote Speakers and displays for community groups

Our team, comprised of a physiotherapist, rehab assistant, and cardiovascular nurse, will work with you and your doctor/nurse practitioner to ensure you get the best possible care for managing your heart disease.

Hours of Operation: Tuesday to Friday, 8:30am - 4:30pm.

Who can participate?

• Adults with Heart Disease ( heart attack, angina, heart surgery, stents, heart transplant)
• Adults at high risk for Heart Disease (diabetes) with high blood pressure and/or high cholesterol and/or smoking

How long is the program?

The exercise program runs 12 weeks (two times per week for 60 minutes).

What education classes are offered?

What should I bring to my appointment?

• Red Hospital Card
• Reading glasses, hearing aid if needed
• ALL medications in their original bottles, prescribed and over-the-counter, including herbals and vitamins
• Prior to the appointment, we request that clients do not drink any caffeine 4 hours prior or any alcohol 48 hours prior to appointment.
• Friend or family to accompany you

How do I register?

Your family doctor/nurse practitioner or cardiologist/internist must refer you to the program. The cost of the program is $100 (for materials). Click here to download a healthfirst referral form.

The cardiac rehabilitation program is located at Ross Memorial Hospital on the first floor entering off of Kent Street.
